Thank you for choosing to use the Betty Health platform. Please read these terms and conditions carefully before accessing or using our platform. By using our platform, you agree to be bound by these terms and conditions.
General Terms and Conditions:
a. Our platform is an online health platform that aggregates 3rd party providers to offer health and wellness services to individuals. The platform is meant to be used as a tool to access these services.
b. Our platform is not responsible for the quality of the services provided by the third-party providers. It is your responsibility to conduct your own research and due diligence to select a provider that best suits your needs.
c. Our platform reserves the right to update or modify these terms and conditions at any time without prior notice. Your continued use of the platform following any changes constitutes your acceptance of the new terms and conditions.
Privacy Policy:
a. Our platform takes the privacy of its users very seriously. We collect and use personal data in accordance with our Privacy Policy, which forms part of these terms and conditions.
b. Personal data includes any information that identifies or can be used to identify you, such as your name, email address, and phone number. We only collect and process personal data that is necessary for the provision of our services.
c. We do not sell or rent your personal data to third parties. We may share your personal data with third-party providers to enable them to provide you with the services you have requested through our platform.
d. We use appropriate security measures to protect your personal data from unauthorized access, use, alteration, or disclosure. However, we cannot guarantee the security of your personal data transmitted through our platform as no method of transmission over the internet or electronic storage is 100% secure.
User Conduct:
a. You agree to use our platform only for lawful purposes and in a manner that does not infringe the rights of, restrict or inhibit anyone else's use and enjoyment of the platform.
b. You agree not to upload, post, transmit, or otherwise make available any content that is harmful, threatening, abusive, harassing, tortuous, defamatory, vulgar, obscene, libelous, invasive of another's privacy, hateful, or racially, ethnically or otherwise objectionable.
c. You agree not to impersonate any person or entity, or falsely state or otherwise misrepresent yourself or your affiliation with any person or entity.
d. You agree not to use the platform to solicit personal information from any individual under the age of 18.
Disclaimer of Warranties:
a. Our platform is provided on an "as is" and "as available" basis without warranties of any kind, whether express or implied, including but not limited to, warranties of merchantability, fitness for a particular purpose, or non-infringement.
b. Our platform does not warrant that the platform will be uninterrupted or error-free, or that defects in the platform will be corrected.
Limitation of Liability:
a. Our platform and its affiliates, directors, officers, employees, agents, suppliers, or licensors shall not be liable for any direct, indirect, incidental, special, or consequential damages arising out of or in connection with your use of the platform, including but not limited to, damages for loss of profits, goodwill, use, data, or other intangible losses.
b. Our platform and its affiliates, directors, officers, employees, agents, suppliers, or licensors shall not be liable for any damages arising out of or in connection with any transaction between you and a third-party provider accessed through the platform.
c. In no event shall our total liability to you for all damages, losses, and causes of action, whether in contract, tort (including but not limited to negligence), or otherwise, exceed the amount of an annual membership for an individual.
Telehealth Privacy Policy & Terms and Conditions
General Terms and Conditions:
a. The telehealth platform is an online platform that enables healthcare providers to offer telehealth services to patients remotely. The platform is meant to be used as a tool to facilitate the provision of these services.
b. The telehealth platform is not responsible for the quality of the services provided by the healthcare providers. It is the responsibility of the patients to conduct their own research and due diligence to select a provider that best suits their needs.
c. The telehealth platform reserves the right to update or modify these terms and conditions at any time without prior notice. The healthcare providers are responsible for keeping themselves informed of any changes and complying with them.
Vendor Responsibilities:
a. The healthcare providers are responsible for ensuring that they have the necessary qualifications, licenses, and permits to provide telehealth services in the jurisdictions where they operate.
b. The healthcare providers are responsible for obtaining informed consent from their patients before providing telehealth services.
c. The healthcare providers are responsible for maintaining the privacy and security of their patients' personal and health information in accordance with applicable laws and regulations.
d. The healthcare providers are responsible for providing their patients with accurate and complete information about the telehealth services they offer, including the risks, benefits, and limitations of telehealth services.
Patient Responsibilities:
a. The patients are responsible for providing accurate and complete information about their health and medical history to the healthcare providers.
b. The patients are responsible for ensuring that they have the necessary technology and equipment to access the telehealth services, including a reliable internet connection and a compatible device.
c. The patients are responsible for complying with the instructions and recommendations of the healthcare providers during telehealth consultations.
Disclaimer of Warranties:
a. The telehealth platform is provided on an "as is" and "as available" basis without warranties of any kind, whether express or implied, including but not limited to, warranties of merchantability, fitness for a particular purpose, or non-infringement.
b. The telehealth platform does not warrant that the platform will be uninterrupted or error-free, or that defects in the platform will be corrected.
Limitation of Liability:
a. The telehealth platform and its affiliates, directors, officers, employees, agents, suppliers, or licensors shall not be liable for any direct, indirect, incidental, special, or consequential damages arising out of or in connection with the use of the platform or the provision of telehealth services by the healthcare providers, including but not limited to, damages for loss of profits, goodwill, use, data, or other intangible losses.
b. In no event shall the total liability of the telehealth platform and its affiliates, directors, officers, employees, agents, suppliers, or licensors to the healthcare providers and their patients for all damages, losses, and causes of action, whether in contract, tort (including but not limited to negligence), or otherwise, exceed the amount paid by the healthcare providers to the telehealth platform for the use of its services.
Further disclosures and refund information:
Membership: Effective upon receipt of eligibility date (the 1st of the month following sign up, unless otherwise determined in writing). Betty Health is not insurance; doesn't meet ACA requirements; does not pay claims of any type or reimburse expenses. There is a 30-day cancellation period, minus a $25 application fee and minus $85 per telehealth visit. If no telehealth visits are used, $229+tax minus $25 application fee will be refunded back to the card or method you used for payment. The same amount will be refunded minus $85 per telehealth visits. If 3 visits are completed, you will have exceeded the entire cost of the membership so zero refund will be available (you might as well keep your membership and let us know what we can do to improve the program for you). FOR UT, AR, TN residents: Refunds for UT, AR, TN residents if canceled in 30 days (with application fee). MD residents: Refund for canceled membership within 30 days (application fee of $25) upon cancellation.

Discount program is at select providers; no direct payments. Members pay with discounts and are responsible for all charges. Providers subject to change; programs vary by state. Providers and locations may be removed. Discount program may be canceled or modified. Discount company doesn't reimburse providers or has any responsibility for care quality. Benefits not valid with other plans. Prices and discounts may change at any time. Providers may offer lower prices to the public. Members pay the lower price between discount and public price. Savings based on provider's fees and varies by location and services/products. Referral plan, no care warranties or guarantees. Providers are responsible for treatment.

QualSight has more than 1,000 locations, so members can choose the provider and the LASIK procedure that meet their vision care needs.
QualSight is contracted with credentialed and experienced providers who have collectively performed more than 7 million procedures.
The QualSight program is not an insured program.
Superior Vision
Members save 5% to 30% off the retail price of eyewear with the Superior Vision discount program. Members are eligible for discounts on exams, eyeglasses, contact lenses, and LASIK at more than 40,000 participating provider locations. Comprehensive eye exams can help detect signs of serious health conditions like glaucoma, diabetes, high blood pressure, and high cholesterol.
Careington Terms and Conditions
THIS PLAN IS NOT INSURANCE and is not intended to replace health insurance. This plan does
not meet the minimum creditable coverage requirements under M.G.L. c.111M and 956 CMR 5.00. This
plan is not a Qualified Health Plan under the Affordable Care Act. This is not a Medicare prescription
drug plan. The range of discounts will vary depending on the type of provider and service. The plan does
not pay providers directly. Plan members must pay for all services but will receive a discount from
participating providers. The list of participating providers is at https://bettyhealth.sourcesolutions.com. A written list of

Group Free--Terms & Conditions 2.19.2020
Membership and Renewal Conditions: By joining a plan for yourself or on behalf of a minor child for whom you are a parent or legal
guardian, you confirm that you are at least 18 years old and have read and agree to the terms and conditions of the plan. This plan will
automatically renew at the end of your membership term.
Termination Conditions: [Group Name] and Careington International Corporation (Careington) reserve the right to terminate plan
members from its plan for any reason.
Cancellation Conditions: Please notify [Group Name] if for any reason you are dissatisfied with the plan and wish to cancel. [Group
Name] will accept and cancel program memberships at any time during the membership period.
Description of Services: Please see the enclosed materials for a specific description of the programs included in your plan.
Limitations, Exclusions & Exceptions: This plan is a discount membership program offered by Careington. Careington is not a
licensed insurer, health maintenance organization or other underwriter of health care services. No portion of any provider’s fees will be
reimbursed or otherwise paid by Careington. Careington is not licensed to provide and does not provide health care services or items to
individuals. You will receive discounts for services at certain health care providers who have contracted with the plan. You are obligated
to pay for all health care services at the time of service. Savings are based upon the provider’s normal fees. Actual savings will vary
depending upon location and specific services or products purchased. Please verify such services with each individual provider. The
plan’s discounts may not be used in conjunction with any other discount plan or program. All listed or quoted prices are current prices
by participating providers and subject to change without notice. Any procedures performed by a non-participating provider are not
discounted. From time to time, certain providers may offer products or services to the general public at prices lower than the discounted
prices available through this plan. In such event, members will be charged the lowest price. Discounts on professional services are not
available where prohibited by law. This plan does not discount all procedures. Providers are subject to change without notice and
services may vary in some states. It is the member’s responsibility to verify that the provider participates in the plan. At any time
Careington may substitute a provider network at its sole discretion. Careington cannot guarantee the continued participation of any
provider. If the provider leaves the plan, you will need to select another provider. Providers contracted by Careington are solely
responsible for the professional advice and treatment rendered to members and Careington disclaims any liability with respect to such
matters.
Complaint Procedure: If you would like to file a complaint regarding your plan membership, you must submit your complaint in writing
to: Careington International Corporation, P.O. Box 2568, Frisco, TX 75034. You have the right to request an appeal if you are
dissatisfied with the complaint resolution. After completing the complaint resolution process, if you remain dissatisfied you may contact
your state insurance department.
Terms and Conditions for Betty Health's Early Adopter Lifetime Access Platform
Please read these Terms and Conditions ("Agreement") carefully before accessing or using Betty Health's Early Adopter Lifetime Access Platform ("Platform"). This Agreement sets forth the legally binding terms and conditions for your use of the Platform.
Acceptance of Terms
By accessing or using the Platform, you acknowledge that you have read, understood, and agree to be bound by this Agreement. If you do not agree with any provision of this Agreement, you must not access or use the Platform.
Description of the Platform
The Platform is an early adopter program provided by Betty Health, granting users lifetime access to its services, features, and benefits, subject to the terms and conditions outlined herein.
Eligibility
To be eligible for the Early Adopter Lifetime Access, you must meet the following criteria:
a. You must be at least 18 years old or the legal age of majority in your jurisdiction.
b. You must provide accurate and complete information during the registration process.
c. You must agree to comply with this Agreement and any other applicable policies provided by Betty Health.
Early Adopter Lifetime Access
a. By joining the Early Adopter Lifetime Access program, you will receive lifetime access to Betty Health's services, features, and benefits as outlined in the program details.
b. Betty Health reserves the right to modify, enhance, or discontinue any services, features, or benefits provided through the Platform at its sole discretion.
c. The Early Adopter Lifetime Access is non-transferable and cannot be sold, assigned, or sublicensed to any other individual or entity.
Payment and Fees
a. The Early Adopter Lifetime Access program is an annual price guaranteed to remain the same every year for life, with the possible exception of a 3% inflationary increase, which we will provide 60 days notice prior to annual renewal
b. You agree to pay all applicable fees promptly and in accordance with the payment terms provided by Betty Health.
c. Betty Health does not intend to increase the price of the Early Adopter Lifetime Access unless necessary. However, in the event of inflationary circumstances, Betty Health reserves the right to implement a maximum of 3% annual price increase solely for inflationary purposes.
User Conduct
a. You are solely responsible for your use of the Platform
b. You agree not to engage in any activities that may violate applicable laws, infringe upon intellectual property rights, or harm the reputation or integrity of Betty Health or its users.
c. You must not use the Platform for any unauthorized commercial purposes, including solicitation or advertising.
Intellectual Property
a. All intellectual property rights related to the Platform, including but not limited to trademarks, logos, copyrights, and trade secrets, are the exclusive property of Betty Health.
b. You may not reproduce, modify, distribute, or create derivative works based on any content from the Platform without the prior written consent of Betty Health.
Privacy
a. Betty Health is committed to protecting your privacy. Please refer to our Privacy Policy for detailed information on how we collect, use, and safeguard your personal data.
b. By using the Platform, you consent to the collection, use, and disclosure of your personal information as described in our Privacy Policy.
Disclaimer of Warranties
a. The Platform is provided on an "as is" and "as available" basis without any warranties, express or implied.
b. Betty Health does not guarantee that the Platform will be error-free, secure, or uninterrupted. You acknowledge that the use of the Platform is at your own risk.
Limitation of Liability
a. To the maximum extent permitted by applicable law, Betty Health shall not be liable for any
